Tuck-in volunteer's will contact Hospice patients/caregivers to maintain continuity of care in an effort to preclude any emergencies. Tuck-in Volunteer will report to the Volunteer Coordinator.

Responsibilities Include:

  • Contact Volunteer Coordinator at the beginning of each week to inquire which patients/caregivers need to be contacted.
  • Telephone each patient and complete Tuck In form.
  • Report identified needs to appropriate staff.
