This program is intended to support unmet client needs in relation to transportation to medical appointments. Medical transportation will be offered to clients who are aged 60 or older and unable to perform at least two (2) activities of daily living or require substantial supervision due to a cognitive or other mental impairment; as well as, to disabled individuals 18 and older with a caregiver aged 60 and over.
