ORGANIZATION: The Health Trust

Better Choices, Better Health Program is an evidence-based chronic disease self-management program developed by Stanford University. The program offers 6 highly interactive workshops where people learn many tools to lead longer and healthier lives while managing a chronic health condition.

Want to lead your family, friends, and neighbors in making better choices and gaining better health? We are looking for YOU -a good future health leader to do just that. Join us in making a difference in your community!

 Do you have a chronic health condition or are you a caregiver for someone who has a chronic health condition?
 Do you feel comfortable speaking in public?
 Are you available Tuesdays December 4th & 11th and Wednesdays December 5th and 12th?

The training fee is $500 with a $50 deposit covering the 4 day curriculum, the 2016 Leader’s Manual, 2012 Better Choices, Better Health Book and CD. This fee will be waived and deposit returned upon commitment to facilitate 2 workshop series per year after completing the training.

To complete and confirm your registration, mail in a check or money order for a $50.00 deposit to the address below. This deposit will be refunded after you are certified.   • Attend the 4-day training and
  • Complete 2 workshops (Each workshop is 2.5 hours each week, with 1 hour of preparation time each week) for a total of 12 weeks (approximately 42 hours).
