The Housing Case Manager will work in conjunction with our in-house Clinical Team to help youth who are aging out of the Child Welfare system to find and sustain permanent housing. The Housing Case Manager will partner with youth to create a transition plan that includes: individualized short and long term goals, financial literacy, and emotional intelligence. The Housing Case Manager will also provide aftercare services with each exiting youth so that the youth have support in maintaining their housing.
Youth Oasis' Diversity House is a Transitional Living Program for 16-17 year old youth in Foster Care. Youth Oasis is committed to partnering with youth in Foster Care to provide the wrap-around supports needed for youth to successfully bridge out of homelessness into permanent housing situations. We are looking for a person that has interest in our population and expertise in providing unbiased care within an affirming environment. Ideally, the Housing Case Manager has an extensive knowledge of area resources and systems and will be able to work extremely well within a diverse, multi-disciplinary team.

Our mission at Youth Oasis is to provide necessary resources and support services to at-risk youth ages 10-21 in order that they may transition to become successful citizens in our community.
Description
Youth Oasis consists of two programs designed to assist youth in crisis.
Our emergency shelter is a 24-hour emergency facility for runaway, homeless youth ages 10-17. We provide immediate shelter, food, clothing and counseling for youth with no where else to turn. We assist families in working through difficult issues in order that they remain intact.
Our transitional living program supports consumers, ages 16-21, with residential living, staff support, and direct connections to services in the Baton Rouge area. Our goal is to instill independent living skills in order that they become self-sufficient.
