Agape Hospice & Palliative Care's mission is to support families and provide a safe, compassionate, comfortable environment that enhances the lives of our patients so they may leave this world peacefully.
Description
Agape Hospice promotes and embraces a palliative care approach to individuals diagnosed with a life-limiting illness as well as supportive services to their loved ones. Physical, social, spiritual and emotional care and support is provided during the last stages of illness for the patient and during bereavement for the family and loved ones. Hospice care is provided by an interdisciplinary team of professionals consisting of the patient's physician, hospice staff and volunteers. Hospice and palliative care is a continuum of care ranging from intermittent home care to inpatient care with emphasis on maintaining the patient at home in their most familiar, comfortable, and least restrictive environment.
