Heartland Hospice ProMedica provides quality care to patients with a limited life expectancy, as well as support to their families and caregivers. We enhance the quality of life of those we serve through palliative and specialized supportive care designed to meet the physical, psychosocial, and spiritual needs of the patients and caregivers. Hospice care is provided by using an interdisciplinary approach in delivering care to patients--either in their own homes or extended care facilities--while working cooperatively with other health care providers and other community resources.
