Social Workers: Students will be able to become involved with social work duties such as intake, working with local community contacts to provide services, advocacy, clinical emotional support, and all the paperwork that is involved in maintaining a patient on hospice.

Psychology and HDFS students: Interns will be albe to become involved with a plethora of disciplines offered at Crossroads. Interns have the oppurtunity to shadow social workers, nurses and bereavement department as well as going out into the field to visit their own patients.

Art Therapists: The interns will be able to provide individual sessions and group experiences for the patients and CRH staff. We have had other Art Therapy interns who have worked here, but each intern is given the opportunity to create a program that is individually chosen by the student and then supplies are purchased if needed.

Music Therapists: The students will be working with patients individually and their facilities and individual homes. The interns will also be helping to develop a music program at Crossroads Hospice. Music is a very important aspect of care for Hospice Patients. Music allows the individual to connect with patients and provide emotional support with verbal and non-verbal patients.

The interns will also have the opportunity to interface with a multi-disciplinary team comprised of a Medical Director, Nurse Practitioner and RN nurse case managers, Social Workers, Chaplains, Bereavement Specialists, Volunteer Manager and volunteers. The supervisor of the students has a MSW and is a Licensed Social Worker.

We have patients in Delaware, Chester, Bucks, Montgomery, and Philadelphia counties.

Crossroads goal is symptom management and pain control. Hope of remission or cure is never abandoned.The focus is on keeping the patient pain free and giving them a good quality of life for whatever time they may have left with us.

Description

Crossroads cares for patients with life limiting illnesses as well as provides support on many levels to loved ones and caregivers. That care includes the psychological, social and spiritual components of hospice.

Crossroad's staff is available around the clock to consult with the family to make after hour visits as appropriate.

For those patients at home, Crossroads has a respite care program, which places the patient in a temporary facility and allows the family and friends some time away for some much needed rest.

Crossroads also provides a "watch" and "continuous care" program when our patient is most critical.
