Responsible to assist in:
The access of Mexican nationals and their families who visit the Mexican Consulate to receive free health screenings and health related information/education. When necessary, refer them to cultural and linguistically appropriate medical and health care services, without regard to their ability to pay.
Identification of medical insurance programs and complete the necessary forms for participants/clients for successful enrollment in such programs.
Glucose, cholesterol, high blood pressure, and other medical and health screening and/social services.
Appropriate and necessary referrals needed by La Ventanilla de Salud participants/clients.
Provision of information related to health prevention programs: diabetes, cholesterol, high blood pressure, smoke cessation, cancer prevention, dementia, home care/home health, nutrition, services for seniors, etc.
Fill out demographic forms, and reports, as necessary
Enroll in available medical insurance programs by completing the required forms.
Channeling Program’s clients/participants to the medical provider’s network based on their health care needs.
Follow ups with the clients/service users of La Ventanilla de Salud to ensure they have received the needed medical services and assistance.
Other duties as assigned.
