HealthSET provides services to the elderly population through the our monthly Health Promotion Clinics. HealthSET's free Health Promotion Clinics are offered to residents at over 20 low-income senior high-rise buildings in the Denver metropolitan area. Each month, our staff and volunteer nurses and pharmacists give seniors wellness assessments; monitor health problems; provide counseling; educate residents about nutrition and other good health practices; and provide referrals to other area health care resources. These clinics help seniors prevent diseases from escalating to the point where they will no longer be able to live independently. Organized around a model of social interaction, the clinics focus on wellness rather than illness. The clinics have the side benefit of combating loneliness, which in itself can exacerbate health problems. To help residents remember when to come in, clinics are held at a particular building on the same day every month. Many of our clients come regularly to our clinics. A client participates with the clinics an average five times a year.
Duties of the Certified Nursing Assistant or Medical Assistant Position:
Meet and greet clients
Obtain vital signs from clients
Identify when a client needs to see a nurse or advocate for follow up
Document clients’ vitals in the appropriate section in the paper chart
Organize and file paper charts
Set up and break down equipment at clinic
Client Advocacy - For clients with complex needs, there is the Client Advocacy Program. The program provides professional advocacy to support older adults as they strive to remain in their homes. Typical needs include access to financial aid or benefit programs, access to medical, dental and mental health services, care management, in-home services, hearing aids, socialization and housing.

HealthSET's free Health Promotion Clinics are offered to residents at over 24 low-income senior high-rise buildings in the Denver metropolitan area. Each month, our staff and volunteer nurses give seniors wellness assessments; monitor health problems; educate residents about nutrition and other good health practices; and provide referrals to the HealthSET social worker for info & assistance. These clinics help seniors prevent chronic illnesses or other health issues from escalating to the point where they will no longer be able to live independently. Organized around a model of social interaction, the clinics focus on wellness rather than illness. To help residents remember when to come in, clinics are held at a particular building on the same day every month. Many of our clients come regularly to our clinics, though it is voluntary.
Client Advocacy - The Client Advocacy Program provides a professional social work to support older adults as they strive to remain in their homes. Typical needs that a client advocate can refer a senior for include access to financial aid or benefit programs, access to medical, dental, vision, and mental health services, nutrition, in-home referrals, Advanced Directive info, and transportation help.
