Working with minimal supervision and requiring a high level of personal initiative, this position is responsible for providing various services/assistance to families affected by Alzheimer’s disease such as support and understanding, acceptance and validation, education/information, and a safe space for people to share difficult feelings and experiences related to dementia.
Provide information and appropriate referrals in response to requests from families, providers, agencies, etc.
Conducting telephone assessments of need for constituents
Assisting families with problem solving and providing support for all affected
Creating action plans to address the families’ identified needs and concerns
Assisting Family Services Manager with organizing, servicing, monitoring, and maintaining care consultation documents and follow ups and input into databases
