Military Community Connection receives requests for assistance from military service members, Veterans and their families Monday through Friday from 9:00 am - 5:00 pm. We are seeking individuals who can assist with incoming clients to conduct a brief intake assessment with our online form and forward the information to the appropriate Care Coordinator on our staff.
Full training is provided.

Mission Statement
The Navigation Center is Charleston's home for a coalition of service providers working together to provide a hub of accessible, connected, and quality resources for Charleston's most vulnerable population -- individuals experiencing or at risk of experiencing homelessness. Our purpose is to identify gaps in services in the Charleston area and advocate for change. We strive to successfully integrate our homeless populations and their families into their communities through a holistic network of community support agencies that are willing and able to provide exemplary service in a coordinated and deliberate fashion. The Navigation Center coordinates a collective impact model to find community solutions to this type of complex problem through a common agenda, shared measurement, mutually reinforcing activities, and continuous communication.
Description
The Navigation Center works with community partners to engage those experiencing homelessness and at risk for homelessness in the Charleston community. Our team started in 2015 under the name Military Community Connections, bringing experienced case management and professional staff together with a wealth of skill and knowledge to provide the holistic support that a Veteran requires to be a self-sufficient and fulfilled member of society outside of the military. In 2018, we expanded to serve not only veterans but the general population as well--becoming a hub of resources for the Charleston community. We take this duty to our clients and knowing the resources available in the community very seriously.
Over 60 Lowcountry providers currently work with The Navigation Center, offering services and resources to clients in employment, housing and transportation, education, subsistence, health and wellness, spiritual support, mentoring and volunteering etc. Through vigilant communications with each of these provider organizations and understanding their procedures and operations, The Navigation Center succeeds in making intelligent and efficient referrals to lead clients in the appropriate direction for their unique needs.
