Omni Care Hospice

Hospice volunteers provide companionship to people living with a serious illness and help their family caregivers in a variety of ways. Hospices also rely on volunteers to help with office work, fund raising, community outreach and other operational areas. Volunteers provide important services to hospice organizations and the people they serve. Whether it’s providing companionship to a person in the final months and weeks of life, offering support to family members and caregivers, or helping with community outreach and fundraising, the contributions of volunteers are essential to the important work provided by our hospice program. Our hospice relies on volunteer support to provide excellent end-of-life care to each patient and family. By being a hospice volunteer, you can gain great personal satisfaction from knowing that you have made an impact in another person’s life.

Mission Statement

The mission of Omni Care hospice is to provide individualized ethical compassionate care to our patients and families. Our purpose is to meet the physical, emotional and spiritual needs of our families while providing quality comfort filled days to the lives of people with life limiting illnesses and their families. Omni Care Hospice emphasizes total patient and family centered care and palliation of symptoms that may occur during this stage of life. We are here to assist with this part of our patients and families lives, therefore we are all family.

Description

Hospice provides palliative, not curative care to meet the physical, emotional and spiritual needs of the client and family. Hospice focuses on the relief of pain and symptoms which may accompany a life limiting illness. Hospice services are provided primarily in the home by an interdisciplinary team of professionals and volunteers. Hospice services may also be provided to the client in a skilled nursing facility, if that is the home.Hospice services may include nursing, physician care, social work, spiritual and other counseling, physical, occupational and speech therapy, volunteer services, medical supplies and equipment and medications prescribed for the relief of pain and symptoms related to the terminal diagnosis. The Hospice team is not intended to take the place of the family, but to support the primary caregiver and family in caring for the client. The opportunity is always there for the patient and their family to join the Hospice team in making decisions about the type, frequency and intensity of services and techniques used to help. We may review the Plan of Care and the patient and/or caregiver can attend the Hospice team conference to discuss the services and techniques being used in my care. We are there to support you through this phase of your life and will do what we can to aid you and your family.
