The Smart Health Clinic assists patients with chronic illnesses to get follow up care after discharge from the hospital. Community Outreach Volunteers ensure patients are receiving follow up care from their doctor, are monitoring their condition (ie. blood sugar, blood pressure), and have access to their prescribed medications. They refer patients to resources and assist staff in locating needed resources.
