Target Zero helps HIV-positive clients achieve and maintain HIV viral load suppression, significantly reducing their risk of transmitting HIV. The goals of this peer-based special initiative are to 1) improve health outcomes of our HIV-positive clients, 2) reduce the community viral load, and 3) improve our ability to rigorously evaluate the impact of GMHC services. Through this initiative, GMHC will monitor critical health data-Primary Care Status Measures (PCSMs)-for all of the agency’s HIV-positive clients. The Peer Navigator who can offer a safe space to clients, discuss obstacles to treatment, and refer them to appropriate care. The Peer Navigator will directly assist clients in entering PCSM data, answer questions about PCSM collection or HIV care, and offer support.

The following duties are mandatory requirements of the job:

  • Engage with clients to collect PCSM information
  • Support clients in navigating through external patient portals to obtain information
  • Coordinate with medical providers to obtain PCSM information for eligible clients
  • Refer clients to GMHC and external services
  • Enter PCSM information into TREAT (GMHC’s electronic health record)

In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:

  • Personal experience navigating the HIV healthcare system, preferred
  • Outgoing/engaging personality
  • Experience navigating medical patient portals and basic computer skills

Training that will be provided:

  • Volunteer Orientation (2 hours)
  • HIPAA & confidentiality training (1 hour)
  • TREAT training (1 hour)

No specific education or certification required.

Time Commitment:

10 hours/week. Flexible schedule, but Monday-Thursday during lunch are preferable.
