Before a family can begin services, they go through an interview process or intake, so that our staff can get to know who they are, why they need services and the best course of action.
Currently, all of our intake forms are available only in paper form, which adds extra time and effort to the intake process. We are looking to you to help us digitize our intake forms & process so that we can welcome new families into our services with efficiency & ease.
We also are looking to have these forms in multiple languages, such as Spanish, Hmong & Somali to make it accessible to more families.
Steps to Get Started:
- Learn about the intake process and get familiar with the forms families need to complete to begin services.
- Digitize any & all of intake related forms.
- Integrate forms into an internally connected system for staff as well as an external system for families to access.
