Job Summary
Under the general supervision of the Case Manager Specialist, this position is responsible for assessment of seniors in Fort Bend and Waller County for meals on wheels eligibility providing case management, telephone resource and referral, and coordination of services meals on wheels consumers. These duties are always performed with the agency mission in mind. Assesments will be assigned as needed throught the month as referrals come in. Assesments will take approximately one hour to complete in the client's home.

Essential Duties and Responsibilities
In-Home Services Case Management

• Assessment and reassessment of potential clients in their home to determine eligibility for meals on wheels program.
• Provide initial screening of potential clients over the phone
• Provide information and referral services for seniors over the phone
• Reassess clients a minimum of at least every twelve months in the client’s home
• Maintain all client files (HGAC, CDBG, Title XX, etc.) in accordance with the policies and procedures of FBS and funding agencies
• Maintain the allowable number of Meals on Wheels clients
• Coordinate homebound meal delivery services for homebound clients
• Provide monthly service reports to the Case Manager Specialist
• Keep the Case Manager Specialist informed of any changes with contract providers
