UR Medicine Home Care, Visiting Nurse Hospice offers caring and comfort through life's final stage. Dying and death present terminally-ill individuals, their families and caregivers many new and difficult challenges. The Visiting Nurse Hospice and Palliative Care Program supports patients and their loved ones throughout this most difficult time, providing professional home care that meets both medical and emotional needs, promoting as full and comfortable a life as possible.

Our hospice and palliative care professionals are experienced in every aspect of hospice care, from guiding the patient's smooth transition from hospital to home...to providing comfort care with respect and compassion...to supporting family and friends through their bereavement following death.
