AFI Advanced Care/Cascade Springs Hospice is designed with the needs of each individual patient in mind. We have an entire team of health care professionals whose goal it is to support and contribute to our patient's care.
This is why we offer senior home health care and hospice services, as well as personal home care. At Cascade Springs, we believe that a fully supportive in-home health care team approach creates the best environment for our patients’ recovery, whether our patient is recovering from surgery, illness, or injury.
Our high quality of care is reflected in the proven positive outcomes we help our patients achieve.
AFI Advanced Care/Cascade Springs Home Health & Hospice has administrative offices located in Clearfield city: We serve Weber County, Davis County, Salt Lake County
Our volunteer opportunity is to visit with hospice patients. You are welcome (depending on patient ability)to just sit and chat, play a card game, do a puzzle, crocheting, crafting, play music etc.

Description
The AFI Hospice team believes patients need to be enabled to preside over their own dying. The AFI Hospice team center's their focus on patient's priorities and needs. Knowing that freedom and dignity can be impaired by severe physical and emotional distress, AFI Hospice seeks to relieve pain and suffering in the physical, emotional, social, and spiritual realms. AFI Hospice is committed to patient-centered care, professional and timely management of symptoms and comfort. AFI Hospice will be that advocate for patients who choose a peaceful, dignified end of life surrounded by family and loved ones.
