MUST HAVE PRIOR CASE MANAGEMENT EXPERIENCE.
Provides intake, assessment, care planning, referral, and monitoring/follow-up of services provided for individuals with chronic illness. Must have a bachelors degree and two years of case management or human services experience. Individual must be very organized. This is a volunteer position.
