"Navigator" is the program that provides palliative care to patients who suffer from long-term illnesses such as congestive heart failure, ALS, COPD and other illnesses by which death is inevitable yet not necessarily imminent. A patient can be in the Navigator program for years before he/she reaches the point of being hospice-ready. Over 700 patients are currently being served by our Navigator program in hospitals, assisted living facilities, in nursing homes and in their own private residences.
Volunteers are needed to assist with a variety of needs regarding our Navigator population: caregiving, check-in visit, providing assistance with meal prep, transportation to medical appts, running errands, etc. For those not interested in working directly with patients, other volunteer options are available: we are looking for volunteers to do check-in calls with these patients on a weekly basis (these are basically calls to determine that the patient is doing ok, taking their medication and to see if they are in need of medical supplies, etc). Also, volunteers are needed to assist here at Headquarters, located conveniently on Cleveland's east side--to make calls to other volunteers to fill patient requests. If you can spare a couple of hours a week and don't mind making phone calls...if you enjoy the rewards of knowing that you are helping patients by filling volunteer requests, then this could be the ideal volunteer opportunity for you!

Description
Hospice of the Western Reserve provides care for patients where ever they call home, whether that be at home, in a nursing home, assisted living, one of our inpatient units, or in a hospital. Care is provided regardless of ability to pay. We use a team approach where volunteers are of equal importance to all other members of the team.
