Provide direct service to the customers identified by CDPHP that will result in significant health behavior change, and attitudes towards accessing health care. This volunteer position will offer services, information and support to move customers away from seeking episodic disease based care. The goal is to help develop foundations for lifestyles founded on prevention and wellness. Help us address the needs of Rensselaer County residents who are low or non healthcare utilizers!

Description
CEO is a non-profit Community Action Agency that serves the residents of Rensselaer County and the surrounding Capital Region. We serve as a conduit -- a way to connect people with resources related to employment, education, housing, youth programming, early childhood education, and daycare. When people are dealing with obstacles presented by poverty, they turn to CEO. Through the agency's centralized intake -- a one stop approach --individuals and families are considered for several services and programs simultaneously. CEO, the Board of Directors and our staff remain committed to advocating for a community that allows each individual to achieve economic independence.
