The mission of Aviant Hospice is to provide unconditional support and compassion to those needing our care. We are our patients advocates, being the unwavering voice of those needing to be heard. We show respect to those we care for - and to each other - as we are all deserving to feel valued and respected. We educate our patients, families and the healthcare community at large so that we may all move forward through the end of life process, feeling strong and empowered.

Description

At Aviant Hospice we aim to exceed the expectations of our patients and families in providing outstanding care, and promoting autonomy resulting in improved quality of life. Our team is committed to providing comfort and dignity through physical, emotional and spiritual support. As Health Care professionals, we are also dedicated to educating the community about the benefit of hospice. We provide care for our patients in their homes, in assisted living and long-term care facilities or wherever our patients call home.

Aviant Hospice affirms life. It neither hastens nor postpones death but assists the patient and family in living with a life limiting illness. Hospice regards dying as a normal process at end of life which we can provide dignity and comfort for the patient and their loved ones.
