If you are a motivated and enthusiastic individual who wants to make a difference in your local community by helping patients live fully during hospice, then this is the perfect opportunity for you. At Medi Home Hospice, we focus on living. We need your help to promote our values and mission. Specific Duties for this volunteer opportunity include the following:

Provide companionship, respite or emotional support to patient and/or family.

Provide transportation, assist families with household chores, or run errands when requested and appropriate.

Record volunteer activity in accordance with policy and turn in documentation promptly.

Wear proper identification at all times when representing Hospice.

Attend 2 in-services per year for continuing education.

Adhere to safety policies and procedures as outlined.

Adhere to confidentiality policy regarding patient information.

Comply with yearly health requirements.

Work effectively with hospice staff, assigned patients and families, and fellow volunteers.

Contact patient, family and/or caregiver within assigned time frame.

Maintain regular contact with assigned patient.

Communicate all concerns or observations pertaining to patient and/or family to Volunteer Coordinator or Hospice team member.

Utilize special talents or skills upon request, as appropriate.

May make follow-up bereavement contacts with family members but is not required to do so.
