Circle the City is a nonprofit healthcare organization for people experiencing homelessness in Phoenix, Arizona. We have a 50-bed medical respite facility where men and women experiencing homelessness while ill or injured can recuperate with on-site medical care, behavioral health services, and case management to connect them with income, health insurance, and housing opportunities. In the last three years, our respite program has cared for more than 700 patients and, of those who have completed their plans of care, 78% have been discharged to housing situations other than the streets or emergency shelter system. We also partner with Hospice of the Valley to provide end-of-life care.
Outreach volunteers will be responsible for tasks related to patient outreach in underserved regions of the Phoenix metropolitan area. Must be abel to walk around in underserved areas, inform homeless populations about the Parsons Family Health Clinic, offer water to anybody who needs it, answer questions about PFHC, and other duties as assigned.

Description
Circle the City provides a time and place to heal for people recovering from injuries or illnesses who are experiencing homelessness. Our compassionate medical and case management team works to achieve the goals of each patient's plan of care, including post-discharge housing. As a 501(c)3 nonprofit, we partner with individuals, groups, and businesses in order to create a circle of community with the goal of ending homelessness altogether
