Mission Statement

Comfort Care Hospice provides supportive, holistic, comfort care to terminally ill patients and their families, by implementing an individualized plan of care developed by an interdisciplanary team.

The purpose of Comfort Care Hospice is to provide palliative care to teminally ill patients and their families in their home. We will seek to enhance the quality of life for the terminally ill patients as well as assisting their families through the provision of social, emotional and bereavement services.

Comfort Care Hospice seeks to enhance the quality of life for the terminally ill patient and their family members through the provision of social, physical, emotional, spriritual, and practical care services and education.

Description

The Hospice program is designed to fulfill the needs of the terminally ill patient as well as the needs of the primary caregiver and the family members.

Hospice is dedicated to preserving the dignity, integrity, and autonomy of the terminally ill patient. Treatment is focused on comfort, pain control, and symptom management.
