Community Care Social Worker provides assessments and recommendations for the client. The Community Care Social Worker will work with each patient’s medical health team and other professionals to assist in appropriate care through transition and care coordination. Community Care Social Worker will assess patients’ ability to comply with patient’s plan of care and/or disease management needs.
Community Care Social Workers will facilitate and/or manage referrals accordingly to ensure continuity of care and appropriate services

  • Requirements:
    Graduation from an accredited school of social work, LMSW required
    Must be licensed in the state of Texas
    Three years of experience working in a community/outpatient setting or three years of social work experience in an inpatient setting
    Bi-lingual a plus
