Looking to be part of a fun and inspiring committee? Community Health Center of Cape Cod is recruiting members for our Golf Tournament Committee to help us put on a wonderful tournament each fall. All while supporting a great mission: providing access to affordable, high-quality healthcare for all, regardless of ability to pay.
We are looking to add members to the committee in 2020 to help make the event bigger and better. We have a fun committee that helps us plan the event and recruit golfers and sponsors - raising money to provide access to care for those who need it most. We typically hold committee meetings monthly leading up to the event. Community Health Center (CHC) is a patient-centered medical home providing integrated primary care, dental and behavioral health services to the most vulnerable children, adults and families on Cape Cod.
CHC is unique in the fact that we provide care to more than 25,000 individuals within a "complete medical home" model, where multidisciplinary, integrated health services are provided under one roof to assist patients overcome transportation, affordability, financial, linguistic, cultural and other barriers to care. Patients who visit can receive primary care, behavioral health care, optometry, dental services and women’s health care, as well as specialized services and a full-service pharmacy.
