Volunteer Discharge Nurses will serve as a patient educator and case manager to assure comprehensive coordinated care. The Nurse will provide education by means of a written or verbal Clinical summary reviewing all aspects of the visit, vital signs, BMI, lab or diagnostic results. Changes in medication and review of current medications while stressing the importance of compliance. The Nurse will also instruct on any follow-up medical visits and upcoming labs and/or diagnostics while always asking the patient if they have any questions. It is important to discuss and document instruction toward lifestyle changes that will help the patient achieve better healthcare indicators.
The volunteer will be a RN who has a broad medical knowledge of medications, disease processes, and available community resources. Strong assessment skills and an intermediate level of computer proficiency is a plus.
