The Ombudsman Program may be reached by phone at (800) 395-8267 or (937) 223-4613. We work with individuals recieveing long-term care and nursing home residents in Montgomery, Preble, Greene, Clark, Miami, Darke, Logan, Shelby, and Champaign Counties.

Our mission is to promote high-quality, compassionate care for consumers of long-term care services through complaint investigation and resolution, and providing access to unbiased factual information on long-term care issues.
Ombudsman Associates are volunteer advocates who seek to enhance the quality of life of long-term care consumers living in nursing homes and assisted living homes. They educate residents, family members and friends about the resources available to them through the Ombudsman program and about the Residents’ Rights. During the COVID-19 pandemic, our Associate Volunteers are calling nursing homes and engaging residents in conversations about the care and services that they are receiving.
