The mission of Transitions Hospice Care is to minimize the suffering of patients in order to enhance their quality of life, and after death to help the patient's surviving family members to cope with the loss.
Description
Transitions Hospice Care offers care to patients with a life-limiting illness. A multidisciplinary team of professionals initiate an individualized plan of care addressing the medical, nursing, psychosocial and spiritual needs of each patient. We provide services in the home, long term care facilities, assisted living facilities, personal care facilities and in-patient units.
