Hospice of the Western Reserve has a need for telephone volunteers to make weekly check-in calls to patients who are in our "Navigator" program. Navigator is a potentially longer-term program in which patients with chronic illnesses (such as COPD, congestive heart failure, ALS, etc) who are not yet hospice appropriate receive care and support as needed. Callers would make weekly calls on THURSDAYS either from one of our HWR locations or from their own homes to patients to make sure that they are doing ok and that they have enough supplies to get them through the weekend. Each call takes only a few minutes but makes a world of difference in being able to offer better patient-centered care. Do you have an hour a week that you can dedicate to making a difference?

Description

Hospice of the Western Reserve provides care for patients where ever they call home, whether that be at home, in a nursing home, assisted living, one of our inpatient units, or in a hospital. Care is provided regardless of ability to pay. We use a team approach where volunteers are of equal importance to all other members of the team.

Hospice of the Western Reserve serves Northern Ohio with offices in Ashtabula, Cuyahoga, Geauga, Lake, Lorain, Medina, Portage, Stark, and Summit counties.
