Daniel Hernandez Youth Foundation is a 501 c (3) nonprofit organization that was established in 2001 that uses fishing to build self-esteem and self-confidence in underprivileged youth. The Foundation's mission is to engage youth in the opportunity to learn fishing fundamentals, with an emphasis on good sportsmanship, environmental awareness, marine sciences, safety, and fun. Our vision is to help build healthy families and strong communities by involving children in outdoor activities.
The foundation utilizes consent sheets gathered at our events. These forms are used to capture key demographic data such as age, geographical information, and attendance history. Participant information is collected so we can monitor our progress and effectiveness as well have information that can be used when applying for grants.
Each event can gather anywhere from 200-1000 consent forms. In order to maintain all of this data volunteers are needed to input data on to computers.
The forms are very simple information such as name, age, gender, and date of event. We have a program on our computers and all that is need is that you input the data into the system.
