1111 E. Spruce Ave.,Fresno, CA 93720

Saint Agnes is looking for volunteers interested in becoming certified facilitators of Stanford's evidence-based model, the Chronic Disease Self-Management Program (CDSMP).

Volunteers will be trained to learn the facilitation and self-management techniques of this successful program which has been nationally and internationally implemented for over 20 year. After trained, volunteers will have the opportunity to teach these techniques to people in need (either at Saint Agnes or out in the community in churches, schools, community centers, apartment complexes, etc.). The trainings are available in English or Spanish depending on the population volunteers are interested in working with.

This is a great opportunity for resume building and career development. Most importantly, volunteers can apply those techniques in their own lives while helping others in the community live healthier lifestyles.

The training is totally free of cost on the following dates:

Spanish- June 1,2,5, and 6 from 8:30am to 4:30pm

English- June 15, 16, 19 and 20 from 8:30am to 4:30pm

Applications must be submitted by May 1, 2017. Please call 559-450-3770 or email mariana.ramirez@samc.com to request an application.
