A Navigator is a trained member of the Doctors Care Health Access team who works individually with patients/clients experiencing barriers to health care and personal life challenges. Primary goal is to improve access to care, assist with gaps and problems in the health care system, and improve overall cost and quality outcomes. We are looking for a volunteer who is Spanish and English speaking.
Navigators do this by listening to client needs and understanding their abilities/resources as well as challenges. With this understanding, Navigators formulate a plan for using the resources, removing barriers and improving health outcomes.
When needed, this includes building longer term working relationships to solve problems and support patients while they learn to self-navigate the health care system.
