We are looking for 7 people who are willing to be trained as a coach. May we count on you?

The Chronic Disease Self-Management Program (CDSMP) is an Evidence Based course that helps both adults with ongoing health conditions and the caregivers of people with ongoing health conditions.

We are looking for 7 people who are willing to be trained as a coach. Our next training class for coaches is set for four days in early 2017.

Once trained as a coach you will team-teach workshops in your community.

The workshops are taught by trained volunteer coaches; many with ongoing conditions themselves. CDSMP teaches practical ways to deal with pain and fatigue, discover better nutrition and exercise choices, understand new treatment choices, and learn better ways to talk with family and doctors about health. Coaches are needed to team-teach a six-week workshop (classes are 2 1/2 hours each class session). Get the training, team-lead the sessions, and make a difference in the life of a client or a caregiver.

  • Orientation or Training
  • Lead two workshop series within one year.
