Living Well with Chronic Conditions workshops are designed to improve the self-management skills of people living with ongoing health issues, Conditions like diabetes, heart problems, mental health conditions, or chronic pain, can cause those affected to suffer health problems over many years. This program teaches new strategies that will give participants the motivation, and skills needed to manage the challenges of living with a chronic health conditions.
Become a Leader for Living Well with Chronic Conditions workshops.
Workshops are offered statewide through the Juniper network--a centralized hub of community providers delivering evidence-based programs to help people manage chronic health conditions, improve balance and prevent falls, and foster well-being.
Once trained you and a partner leader can lead a 6-week structured workshop in your community; you can determine your schedule for when to facilitate a workshop.
