Job Description: The purpose of this position will be to coordinate patient care from the time the provider leaves the patient room until the patient is checked out. The goals will be to provide a more effective discharge process for patients and a supportive work environment for clinic, increasing patient compliance and decreasing calls made to the office to clarify recommendations. Duties will include: reviewing patient encounter to fully understand provider expectations; ensuring that any orders written for the patient are accounted for and executed appropriately; printing all prescriptions and diagnostic orders (that are not escribed); supplying the patient with any supplies ordered that are not carried in the pharmacy (medications) and do not need to be purchased at checkout (strips and glucometers); reviewing discharge instructions with patient and messaging through Athena text to notify checkout that your patient is "ready for checkout" and escorting patient to checkout when it is their turn. Bilingual (Spanish/English) is a plus.

Description
Christ Clinic services include primary medical care, pharmaceutical assistance and patient education. Christ Clinic provides healthcare access, chronic disease management and preventative care services Monday - Thursday 8:30 AM - 7:00 PM and Friday 8:30 AM - 3:00 PM
In addition, Christ Clinic also obtains donated medications through local doctor’s offices and other national medication donation programs. Volunteers and staff at Christ Clinic also work with local pharmacies who give patients medication at NO or very low cost to the patients. Christ Clinic also helps patients apply for medication assistance from pharmaceutical companies to get name brand medication at no cost to the patients.
Christ Clinic provides chronic disease management and addresses obesity, diabetes and nutrition through patient education classes. Education classes include information about diabetes, maintaining a healthy heart, nutrition classes to promote healthy eating and smoking cessation. As a result of these classes, patients are able to manage their chronic conditions and improve their overall health.
Through these various programs Christ Clinic is able to assist the most vulnerable people living and working in our area. Throughout the year we are able to help numerous people get the healthcare they need.
