Heartland Hospice cares for patients who reside in nursing facilities and private homes. Our volunteers reach out to them as friendly visitors and emotional support. Description
Heartland Hospice provides care to people who have a life-limiting illness of 6 months or less. Services include a team approach to care provided by Registered Nurses, Physicians, Chaplains, Social Workers, Certified Nursing Assistants, Volunteers and Bereavement Counselors. Services are provided in Skilled Nursing facilities, assisted living facilities and private residences in the greater Philadelphia area.
