Community Home Care and Hospice is seeking compassionate hearts and hands for our volunteer program.
Companionship: Visit with patients in their home setting, providing conversation, reading a book, playing a game, watching television, listening to music, or simply just holding a patient's hand.
Patient/Family Support: Provide respite for family caregiver, run errands, meal preparation, light housekeeping, or yard work.
Special Events: Work hand-in-hand with our hospice care team to educate and support the community.
Administrative: Answer phones, clerical duties, supply management, bereavement phone calls and letters.
Share yourself! Your visit can make someone's day. Our volunteers bring joy and companionship to patients and provide much needed support to their caregivers!

Community Home Care & Hospice - Troy is committed to clinical excellence in hospice care. Our skilled professionals and volunteers provide a full-service hospice program customized to address the physical and emotional and spiritual needs of the patient and their caregivers.

Examples of Opportunities include:
- Calling patients and their families
- Writing letters to patients
- Visiting with patients to provide companionship for the patient or a short respite for the family
- Visiting with families to provide chaplain visits, support visits, or bereavement visits
- Visiting with patients to provide companionship for the patient or a short respite for the family
- Performing arts and crafts or bingo activities in nursing facilities or Senior Centers
- Assisting with holiday decorating in facilities, homes, or the community
- Preparing meals for patients and their families
- Hosting birthday parties for patients
- Hosting special requests through the Wishing Well Program (which fulfills end of life wishes for patients)
- Honoring Veterans through the We Honor Veterans Program through a pinning ceremony and celebration

REQUIREMENTS

  • Driver's License Needed
  • Background Check
  • Must be at least 16
  • Orientation or Training
  • Once assigned to work with a family we'd like for you to commit to following them, until they no longer need you. Time frames do vary.
  • Medicare requires a minimum of 8 hours of training for Administrative volunteers and 12 hours for Direct Patient related volunteers. We will provide the training and work with you.
