The Mount Sinai Health Partners Volunteer Program utilizes volunteers to assist with essential, non-clinical tasks; working along side with Social Workers and Care Coordinators, which allow them to focus on a patient’s clinical needs.

Tasks include, but are not limited to:

  • Researching community resources for patients
  • Scheduling appointments and transportation
  • Performing telephone advocacy on patient behalf in regards to community benefits and entitlements
  • Making appointment reminder calls to patients
  • Going on runs to pharmacy or other stores to purchase items for patients
  • Calling insurance companies and pharmacies to verify benefits
  • Performing clerical tasks - faxing, making copies
  • Referring patients to community resources such as Access-A-Ride and Meals on Wheels

Please note there is a minimum commitment of 1 shift of at least 4 hours/week. Additionally, the Volunteer Program office is not open on weekends, therefore only weekdays shifts available. Mission Statement

Mount Sinai Health Partners Care Management Program consists of longitudinal care management programs designed to reduce the number of 30-day readmissions to the hospital. These programs are staffed with social workers and care coordinators who assess the individual needs of patients that are high risk for readmission. During the 4 weeks following discharge, patients are followed closely in the community by their social worker or care coordinator in an effort to improve diet and medication compliance, coordinate patients’ medical care and assist patients in getting to follow up visits. Social workers/care coordinators also address a variety of other social needs including housing, insurance, mental health and substance abuse issues which are thought to be contributing to the individual’s readmissions.

Description

The Mount Sinai Health Partners Care Management Volunteer Program is staffed with volunteers that aid Social Workers and Care Coordinators with a variety of tasks for their patients. Some of these tasks include research, runs to the community to pick up items for patients, calls to providers offices (scheduling, advocacy), calls to patients/family members, connecting patients to necessary resources.

Volunteers must be able to commit to at least 2 shifts of 4 hours/week, for a minimum of 6 months. We operate during regular business hours, 9am-5pm. Prior to volunteering, you will be need to be medically cleared through Mount Sinai Hospital.
