Description
Sojourn Hospice, a program of Healthy Living, Inc., is a hospice serving Santa Clara and San Mateo Counties. We are dedicated to supporting individuals and their families on the journey through the final stages of life. The physical, emotional, spiritual, and mental challenges specific to this experience can be overwhelming for the family and the person with a terminal illness. Simple human kindness is especially valued and appreciated in these difficult times.
