This invidual or family will provide emotional companion support to our patients/families on a one-on-one basis. Volunteer(s) will provide this support either in a patient/family home, long term care facility or assisted lving facility. This support will be one to four times a month as determined by the patient and volunteer, to meet the emotional support needs of the patient and their family.

Description
Hometown Hospice staff and volunteers provide physical, emotional and spiritual support to our patients and their families who are dealing with end of life issues. We provide this care wherever the patient resides (home, nursing home, assisted living)
