Description

Program Overview: Members of AmeriCorps Targeted Outreach Program will provide services to students identified as at-risk for gang involvement and delinquency. Members will serve collaboratively with local Boys & Girls Club programs within the Boys & Girls Clubs of Northeast Florida, the Boys & Girls Clubs of Alachua County, the Boys & Girls Clubs of Lake and Sumter Counties, and the Boys & Girls Clubs of Volusia/ Flagler Counties. Members will be also be responsible for mentoring youth identified by Twin Oaks Juvenile Development's Project Connect Program. This program assists youth transition out of Florida Department of Juvenile Justice residential commitment facilities back into their community as productive, accountable and responsible citizens. Members will meet with Twin Oaks Juvenile Development's Project Connect youth for one hour, two times a month to review the youth's progress towards the completion of their Individualized Transition Plan and assist them in navigating the obstacles faced with re-connecting to local education providers. Member must have: - A high school diploma or its equivalent. - The ability to relate to people from diverse backgrounds. - Excellent communication and organization skills. - Creative and able to adapt to various situations

Member Duties : Members will serve students identified as at-risk for gang involvement and delinquency. Members will assess the specific needs of at-risk students and assist the students in developing an individualized service plan to target their risk factors that lead to poor academic performance, substance abuse, gang involvement or delinquency. Members will also be serving youth identified by Twin Oaks Juvenile Development's Project Connect Program. This program assists youth transition out of residential facilities back into their home community as productive, accountable and responsible citizens. Members will meet with these youth for 1 hour, 2 times a month to review the youth's progress towards the completion of their individualized transition plan and assist them in navigating the obstacles faced with re-connection to local education providers.
