Description

Legacy Hospice is a program of supportive care for persons with terminal illness. Hospice focuses on providing a full range of physical, emotional, social and spiritual comfort to both the patient and family. We strive to enhance the quality of life for the entire family and offer patients the ability to direct their own care.

Our program is primarily based in the home. Many terminally ill patients prefer receiving care in the comfort of their home and with the involvement of family and friends. Care can also be provided in an alternative home, such as adult foster care, an assisted living or nursing facility. Acute care needs may be met at Legacy Hopewell House Hospice Center, in Southwest Portland.
