Hospice Advantage provides the best end-of-life care through an interdisciplinary family support team that supports patients and families. The mission is achieved by our commitment to cornerstone philosophies by: providing quality end-of-life care to patients of ability to pay;promoting patient and family as the center of care; promoting a supportive professional environment; evaluating customer needs to better meet the needs of the individual, referral source and community; increasing access of end-of-life care;supporting communication internally and externally that is timely,respectful and professional; developing and providing innovative state-of-the-art care; providing care in a fiscally responsible manner; utilizing electronic systems to provide enhanced care capability.
