Tuck-in volunteers are available to contact patients/families either by phone for the purpose of obtaining specific information that will assist in preparing patients for weekends.
Duties:
- Review request and assignment with referring case manager and/or volunteer manager.
- Make telephone contact.
- Conduct assessment of needs.
- Provide Volunteer Coordinator with the information obtained according to polices.
- Understand limitations of the role of the volunteer in this capacity and refer to appropriate team members as necessary.
- Initiate communication to appropriate staff when concerns arise.
- Document all interventions according to policy.
- Write notes to staff when they are given a positive comment by a patient or caregiver
Requirements: 4 hour Initial Volunteer Training Class, application, criminal background check, TB skin test, copy of driver's license, proof of auto insurance, confidentiality agreement, and liability release.
Qualifications: Patience, compassion, flexibility, dependability and good active listening skills are needed. The volunteer must be able to put the patient's interests first. Being well grounded and possessing good common sense/reasoning skills is important. Communication and interpersonal skills. A clear, respectful, and easily understandable phone voice. Ability to fill out paperwork and communicate results to Volunteer Coordinator.
Time Commitment: Be willing to make a service commitment of at least 1 hour a week for 6 months. Calls are made on Thursday afternoon or Friday morning.
Reporting: Daily reports of calls and office activities are completed and given to Volunteer Coordinator.
