Tuck-in volunteers are available to contact patients/families either by phone for the purpose of obtaining specific information that will assist in preparing patients for weekends.

Duties:

1. Review request and assignment with referring case manager and/or volunteer manager.
2. Make telephone contact.
3. Conduct assessment of needs.
4. Provide Volunteer Coordinator with the information obtained according to polices.
5. Understand limitations of the role of the volunteer in this capacity and refer to appropriate team members as necessary.
6. Initiate communication to appropriate staff when concerns arise.
7. Document all interventions according to policy.
8. Write notes to staff when they are given a positive comment by a patient or caregiver

Requirements: 4 hour Initial Volunteer Training Class, application, criminal background check, TB skin test, copy of driver's license, proof of auto insurance, confidentiality agreement, and liability release.

Qualifications: Patience, compassion, flexibility, dependability and good active listening skills are needed. The volunteer must be able to put the patient's interests first. Being well grounded and possessing good common sense/reasoning skills is important. Communication and interpersonal skills. A clear, respectful, and easily understandable phone voice. Ability to fill out paperwork and communicate results to Volunteer Coordinator.

Time Commitment: Be willing to make a service commitment of at least 1 hour a week for 6 months. Calls are made on Thursday afternoon or Friday morning.

Reporting: Daily reports of calls and office activities are completed and given to Volunteer Coordinator.
