Mission Statement

To improve the health status, promote independence and enhance the quality of life for individuals and families in Western Michigan by providing high quality healthcare and rehabilitative and supportive services in the home and in the community.

We will accomplish this through partnerships with other providers in a seamless continuum which promotes patient care in the most appropriate, cost effective, and least restrictive setting.
