Mission Statement
New Life Transitions provides transitional services to formerly incarcerated individuals who are motivated to make a new start for themselves. New Life Transitions is a registered non-profit and Charitable Organization in the State of Illinois. New Life Transitions is501(3) c tax exempt non-profit organization.
Description
New Life Transition’s primary focus is to provide reentry opportunities for ex-incarcerated individuals. Secondary focus is to prevent incarceration by addressing at-risk individuals who are likely to be incarcerated (for example, homeless and desperate individuals who may commit crimes to meet their needs).
The primary method for recovery and re-orientation is the RISE program. The RISE Program is based around the "Central Eight" Risk Factors That Place a Person at Risk for Future Criminal Behavior" from SAMSHA’s "Guidelines for the Successful Transition of People with Behavioral Health Disorders from Jail and Prison".
In order to successfully transition into society, one has to learn new behaviors that reduce the tendency towards antisocial behavior, personality patterns, cognition, and as well as poor relationships with family, school and work, and begin patterns through which they are involved in non-criminal pursuits.
In addition, most individuals are dealing with various levels of co-occurring mental health issues and substance abuse. During their transition, they need help in finding and sustaining mental health or substance abuse counseling, medication, and groups. Included in the RISE program is the Genesis Process for relapse prevention.
