Hudson Headwaters has a patient advisory council made up of people who receive care at our health centers. Council members (Patient Advocates) help us understand how patients feel about their care and what they experience in a typical visit. The council meets regularly to discuss common concerns and to offer suggestions. the goal is to improve how patients experience care at Hudson Headwaters. We are currently looking for new members to represent each of our 16 health centers.

Description

Hudson Headwaters Health Network began with a single health center that opened in Chestertown in 1974. At the time, the region north of Glens Falls faced an acute shortage of health care providers. Physicians were retiring; small hospitals were closing. The whole region, home to many people with no health insurance and limited income, faced little or no access to basic health services.

To fill the growing need, health centers were established in Warrensburg, North Creek and Indian Lake. In 1981, with federal funding for community health centers (funding that we still receive today), these four centers officially became the Hudson Headwaters Health Network -- a community-based, state-licensed, not-for-profit health corporation. In each instance, the local health center was made possible by partnership. Hudson Headwaters provided the care; the community provided the facility.

As need for additional care grew throughout the region, so did Hudson Headwaters. We established centers in Bolton Landing in 1985, then Schroon Lake and Ticonderoga in 1993. During the next decade, new health centers were established on Broad Street in Glens Falls, in Moreau and in Queensbury. In the past seven years, Hudson Headwaters has opened health centers in the Town of Moriah (2005), the Albert R. Tucker Clinical Services Center in Warrensburg (2005), Fort Edward (2010) and a second Queensbury facility (January, 2012). Our 15th health center in Champlain, NY joined the Network in October 2012. In January, 2013, we opened our second health center in Queensbury, West Mountain Primary Care.

Hudson Headwaters Health Network staff and programs have earned a reputation for excellence. Recognized by federal authorities and state agencies for leadership in rural health care, Hudson Headwaters has pioneered new approaches to regional health planning and reimbursement. Hudson Headwaters is proud to serve as a demonstration site for new health care delivery strategies, including the Adirondack Region Medical Home Pilot.
