Our Case Managers are providing services funded by various federal, state, and multiple county government grants and service contracts that require considerable documentation requirements before, during and after the delivery of funded services to eligible clients.
A Case Manager's Assistant helps our Case Managers set appointments with clients, greet clients before and after their visits with Case Managers, and complete the documentation requirements in order for that service to be paid for by its respective grant funder(s). Assistants may also answer the office's mail line and direct callers to the appropriate staff person, help set-up or put-away office items before and after client group meetings, and perform other basic office duties.

Mission Statement
Foothill AIDS Project seeks to improve the lives of those people affected by HIV/AIDS by providing a continuum of comprehensive support services, and FAP seeks to stop the spread of HIV with meaningful client and community education programs.
Description
Annually, FAP delivers care and compassion to 800+ men, women and adolescents infected or affected by HIV/AIDS across the Inland Empire from its offices in the cities of Pasadena, Claremont, San Bernardino, Riverside and Hesperia (high desert). 90% of clients live in poverty, 70% struggle with other life challenges in addition to HIV (diabetes, COPD, addiction, depression, etc.), 65% of clients are Latino or African-American, and 35% of clients are women living with HIV/AIDS, many raising small children.
Eligible clients may receive the following major services, including access to benefits, linkage to care, social work case management, medical case management, one-on-one counseling, facilitated support groups, recovery support programs, client educational forums, housing support, mail-order pharmacy, and food assistance, among other services.
