We are in the initial start up phase of a non-profit organization and need someone to help with grant writing and fund proposals.. The Grant Writer will conduct the full range of activities required to prepare, submit, and manage grant proposals to foundation and corporate sources. This includes researching, compiling, writing and editing grant applications.

• Develops and prepares proposals and grant applications according to guidelines; complies with all follow-up grant reporting as required by foundation/corporate donors.
• Acquires and maintains sound knowledge of the organization, and uses that knowledge and understanding to better comprehend all projects and programs for which grants will be sought and to recommend the seeking of grants.
• Assists with other grant writing projects as needed.

About Us:

Personal Physician Care Community Center operates one the newest comprehensive community health initiatives that aim to improve health and reduce the burden of chronic disease to the residence of Delray Beach and surrounding areas. The center uses evidenced based strategies to create programs for healthy life style interventions that make it easier for people to make healthier choices and take control of their health.

Living Well with Chronic Conditions is a workshop for people with lifelong health conditions, such as heart disease, arthritis or diabetes. It works: Participants have less pain, more energy and fewer hospitalizations. The workshops teach participants how to take good care of their conditions, and create a supportive environment and a sense of community. Some of the topics covered in the workshop include pain and fatigue, eating well, fun ways to get active, and communication with doctors and family members.

Chronic diseases--heart disease, stroke, cancer, diabetes and arthritis--are among the most common, costly, and preventable of all health problems in the U.S, according to the Centers for Disease Control and Prevention. Half of all Americans have at least one chronic disease, and seven in ten U.S. deaths each year are from chronic diseases.

Much of this could be prevented or lessened by creating communities where everyone can eat well, move more, avoid tobacco use and obtain regular health screenings. In the meantime, for the many people who already have chronic diseases, and for those diseases we can’t entirely prevent, we must help Floridians live better with their health conditions.
